Extensor mechanism sparing para tricipital posterior approach to the distal humeral shaft fractures: An alternative approach for fracture treatment

Introduction: Generally Anterolateral and posterior surgical approaches are most commonly used for fractures of middle thirds distal third the humerus. In approach, usually splitting triceps is done exploring We studied an alternative, reflecting para tricipital approach which spares extensor mechanism. using it lower extra articular humerus fracture to achieve good exposure so as avoid muscle gaining early ROM range motion.Materials Methods: 15 skeletally mature at our center treated by Triceps sparing with minimum follow up a period 6 months. They were functionally assessed MAYO score, measurements radiological union. Mean duration was 10 months from 6-14 months.Results: The average Mayo score 95 indicating excellent performance mean functional 132 degree 120-140 degree. Only 1 patient had complication infection not related approach.Conclusion: (TRA) valuable option ORIF in shaft fractures. mechanism-sparing Paratricipital invaluable fixation without negative effects on strength. clinical outcome parameters series revealed maintenance strength compared contralateral side. This gives us liberty mobilize elbow joint actively well passively also avoids complications adhesions.
